Welcome to JB Morton Health - A Gateway to Health.
We are a mobile practice of holistic medicine practitioners fully equipped and ready to meet the needs of our patients and clients. Please read through the information below and then complete our electronic Patient Registration form if you are a new patient.
Our patient's health needs come first and, therefore, some treatment methods may not be covered by your insurance plan. Read our Insurance section below to understand your patient responsibility. Payment is expected at the time of your visit.

Insurance
Q: Can I use insurance to cover my visit?
A: No. But, in some cases, health savings account funds may be used to cover naturopathic visits, or the visits may be counted towards your deductible. It is the patient's responsibility to figure out the details with your insurance company. We can provide documentation with the proper codes for you to submit your visit to your insurance company. If you have direct questions, please give us a call. You may also need to contact your insurance plan.
New Patient Registration
If you are a New Patient, please complete the electronic registration form below prior to your initial consultation. This will allow us to gather the necessary information needed before scheduling. Existing patients do not need to fill out the form.
